United States Flags

One of the services I provide to Hawaii constituents is the opportunity to have a flag flown over the United States Capitol. You can obtain a flag by filling out the request form below and mailing it, along with proper payment, to my office. (Sorry, I cannot accept flag requests via e-mail.) A flag certificate will accompany the flag verifying that the flag was flown over the U.S. Capitol. This certificate will also contain information provided by you when you fill out the form below as to whom and when the flag was flown. Flag Form: PDF/Word Document

Flag request should include the following:

1. Size of flag

2. Name of recipient of the flag and certificate

3. Occasion for presentation such as birthday, retirement, etc.

4. Date to be flown. This is the date listed on the certificate (actual date that the flag is flown). The flag office estimates a 3-week turn around time. The flag/certification should be received with 3 weeks FROM the flying date.

   Occasionally, meaningful days are selected for flying or often the requester will ask that the flag be flown on the earliest date.

   Flags are flown 365 days a year.

5. Mailing instructions:
   Name, complete mailing address, the date the completed request is to be received by, and a contact phone number.

6. Please allow four to six weeks for processing and delivery. Please note that requests must be made in writing as payment is necessary to process the request.
